The Los Angeles County Homeless Outreach Portal (LA-HOP) is a new, straightforward system designed to assist people experiencing homelessness with outreach services.
The process utilizes outreach workers who have a variety of skills including homeless services navigation, mental health first aid, motivational interviewing, and trauma-informed care. Some teams have physical health, mental health, or substance use professionals.
Outreach teams start by building trusting relationships and determining the needs of people living on the streets. Efforts can be as simple as helping someone experiencing homelessness get an ID card or as complex as helping to meet medical and mental health needs. However, the ultimate goal of outreach services is to help people experiencing homelessness move off of the streets and into a permanent home.
